I acknowledge by signing this form that Baleigh Cerv Photography is not responsible for accidents, lost or stolen items or injuries. By submitting contract Baleigh has permission to photograph above stated client and use images for advertising and social media purposes. Turn around time is 2 weeks from initial photo session date. Deposits are required with booking to secure date. Session fee must be paid before sessions begins. I acknowledge that the rights to my photos are not included in my package and are additional unless stated differently.
